Thank you for choosing to register with the Liberty Bridge Road Practice
In order to register you need to live in the London Borough of Newham.
If you live outside this area, your registration will be considered Out of Area and you will not have access to certain domiciliary services, such as home visits from a GP. Please contact us to discuss what it will mean for you to be an ‘out of area’ patient or click on the document below for more information.

Online Registrations
To register online, please click on the blue circle below.
Please note that we cannot currently accept online registrations for children. You need to be over 16 to use this service. Children are very welcome to register with us, but a parent or guardian needs to visit the practice to complete the form in person (please see section below).
Please complete all the fields on the registration form and the medical summary. Incomplete forms will considerably delay your registration.
We may need to contact you to verify some of the information that you have provided or to ask for additional information. Please ensure that your contact details are correct.
Tip: Including your previous address is vital for processing your registration. Please ensure to fill in your previous address
Registration’s will be processed within 10 working days and you will receive a phone call to confirm your registration. If you have not heard from us after this time, please contact us. If you have an urgent medical problem and need to be registered sooner, please let one of our receptionists or ad
You will need to attend the surgery within 30 days to:
- Sign the registration Form
- Sign the Consent Forms
- Have a New Patient Health Check
Registrations in Person
You are welcome to attend the Practice at anytime during the opening hours to register with us. You will be asked to fill in a registration document and to sign a few consent forms. To save time, you may wish to print these forms in advance.
Children (Under 18) registration form
You may be asked for proof of address or proof of Identity. This is only to ensure that the correct spelling of your name and address is included on your registration documents. Having a proof of ID or address is NOT a pre-requisite to your registration.
Once you have handed in your registration and consent forms, you will also receive a New Patient Information Pack and book a New Patient Health Check appointment.
Temporary Registrations
If you are a patient who is not registered with the practice but are suffering from an urgent medical condition, we can register you as a temporary patient in order to give you advice and treatment until you can be seen by your own GP. You will be required to fill out a short form at reception and we will do our best to ensure you are seen by a healthcare professional as soon as possible.
Please note that as a temporary patient we may not be able to refer you to secondary care or community services.
